Fertility Insurance

While Boston IVF – The Arizona Center is contracted with most major insurance companies, infertility benefits are not considered “standard care” and covered benefits will vary greatly between carriers and individual policies. We recommend you call your insurance company to check your benefits to protect yourself from charges that may not be covered by your plan. If you do have infertility coverage, you should obtain written verification of your benefits.

We know that verifying infertility benefits can be confusing, so we’ve created a worksheet with some helpful questions to ask your insurance company. Questions Regarding Coverage to Ask Your Insurance Company
